Congratulations on enrolling your child.
New enrollment for K-12 students takes place at the District 194 Administrative Office. The Student Information Services staff are available year-round to support parents and guardians with registering their children for school.
Enrollment packets are available at the District 194 Administrative Office or can be downloaded from the District website below. Because some forms require signatures, parents and guardians are asked to return all completed forms to the District 194 Administrative Office.

Please note: Families who live outside District 194 will need to apply for Open Enrollment prior to the start of school and before enrolling their children. Families who live inside District 194 and would like to apply for their child to attend a different District 194 school will need to apply for Intra District Transfer.

Bring the completed enrollment packet to the District 194 Administrative Office.
Parents and guardians are also asked to provide the following information in-person:
- Proof of student’s age and name – birth certificate, passport, etc.
- Emergency contact information for parent/legal guardian
- Proof of parent/guardian residency – utility bill, rental agreement, etc.
- Name, address and telephone number of most recent school attended by the student
