Workers’ Compensation 

If a staff member is injured while at work, report the incident immediately using the online First Report of Injury. Don’t wait. The online report will automatically update the district workers’ compensation representative and ensure the supervisor is aware as soon as possible. Failure to report the injury within time-frames set by law may result in penalties to the district.

Initial steps if a staff member is injured while at work:

  • If emergency or injury, ensure staff member has the necessary medical attention immediately
  • Complete of the First Report of Injury form to the best of your ability (must be completed by a staff member other than the injured employee).
    • First Report of Injury forms must be submitted within 24 hrs of injury.
  • Reporting staff member supplies the Employee Information Packet with the CorVel Patient ID Card to the injured employee.
  • Instruct the employee to contact CorVel Managed Care: 612-436-2500 or 877-703-4241
  • Supervisor must complete the Supervisor’s Investigation Report within 24 hours of your work-related injury.
  • Partner with Emily Herman to monitor employee’s return to work and any work restrictions and/or necessary accommodations.
