Teachers On Call (TOC) - Substitute Requests

Lakeville Area Schools is pleased to announce our new partnership with Teachers On Call, a Kelly Services Company, as our comprehensive provider of our substitute workforce. Your access to AESOP/Absence Management will remain the same. You can still request a substitute by entering your request in to AESOP. However, if you need to call in your request for a substitute, you can now contact the Substitute Services team at Teachers On Call. See the contact information for support to request a substitute.   • Approve sub timesheet
  • If sub not assigned, TOC will actively search to fill the vacancy

10-14 Days

  • Complete the Extended Leave of Absence Form
  • Follow the steps listed on HR intranet for extended leaves
  • Enter leave into AESOP/Absence Management as soon as possible
  • Under Substitute Required field, select 'Yes' if you need a sub
  • Update your OM and HR when:
    • your leave begins
    • if your return to work date changes
    • you officially return to work
  • If you have a specific sub*, assign the sub to your leave (ensure your OM is aware)
  • Point person at school to ensure your substitute is informed
  • Approve sub timesheet
  • If sub not assigned, TOC will actively search to fill the vacancy
  • Available to help employee submit leave and answer questions

 

15-29 Days

  • If sub not assigned, TOC will actively search to fill the vacancy
  • Available to help employee submit leave and answer questions

30+ days (licensed)

  • Complete the Extended Leave of Absence Form
  • Follow the steps listed on HR intranet for extended leaves
  • Enter leave into AESOP/Absence Management as soon as possible
  • Under Substitute Required field, select 'No' if you need a sub and in Message state "LTS under contract"
  • Update your OM and HR when:
    • your leave begins
    • if your return to work date changes
    • you officially return to work
  • If you have a specific LTS in mind, please inform your OM for consideration during the hiring process
  • Work with HR to hire LTS via Job Posting and New Hire forms
  • Point person at school to ensure your substitute is informed
  • Approve sub timesheet
  • Available to help employee submit leave and answer questions
  • If the LTS submits a leave, TOC will support based on the grid levels above

*All substitutes must be employees with Teachers On Call. In the implementation stage with Teachers On Call, we will transition all current district substitutes. If you hear from a substitute who has not yet enrolled with Teachers On Call or has questions, please encourage them to contact the Teachers On Call office at 800-713-4439.

  • You are not required to create a Frontline Account for phone access. You automatically have phone access. In order to access your account by computer or mobile app, you must create a Frontline Account using your personal invitation email.

Phone Access

  • Call toll free: 1-800-942-3767
    • ID Number: Your personal phone number
    • PIN: Your ISD194 employee number
  • Follow the voice menu to enter and manage absences and access other features
  • Tip: Call in to check the computer recording of your name and title
  • IMPORTANT: When entering your absence, wait until you receive a confirmation number before you terminate the phone call. Your transaction is not complete until you receive a confirmation number.

Computer Access

  • You will receive a personal invitation email with the subject line: Lakeville Area Schools invites you to Absence Management
    • If you have an existing Frontline ID Account, select "Sign in with your Frontline ID"
  • New to Frontline
    • Select "Create a Frontline ID"
    • Enter a Username - please use your district email (last four letters of your last name + four numbers. Example: herm1980)
    • Create your password
    • Enter your email address
    • Select "I accept the terms and conditions" box
    • Select "Create Frontline ID" and you're in!

FrontlineMobile App Access

After you have created your Frontline ID

  • Go to your mobile app store and download the Frontline Education Mobile App 
  • When the App opens you will be prompted for a four-digit invitation code: 7354
  • Enter your Frontline ID credentials to sign in

Contact Information

All medical leaves and work restrictions are processed by Human Resources. Medical documentation should not be sent directly to Supervisors. Instead, send all communications and documents directly to Jeff Tickle.

Steps for Each Process

Complete the steps outlined below. If additional steps are required, Human Resources will provide additional instructions. Be sure to review your Contract for available benefits. For any questions throughout the process, contact Jeff Tickle.

Medical Leaves

  • Complete the Leave of Absence form

  • Send Medical Documentation to Human Resources, Attention Jeff Tickle.

  • Enter the appropriate “reasons” under your leave reporting system (either Employee Access Center or AESOP).

  • Confirm your first day off and confirm your return date to Jeff Tickle via email at jeff.tickle@isd194.org.

  • For personal medical leaves, you are required to submit a Return to Work Authorization to Human Resources, Attention Mia Long, prior to your return to work.  


Work Restrictions

  • All work restrictions must be processed by Human Resources prior to employee’s return to work.
  • Send medical documentation outlining work restrictions to Human Resources, Attention Jeff Tickle.
  • All medical documentation with set work restrictions must include the duration of the work restriction. If TBD, note must state the next follow-up date for further evaluation of work restrictions.
    • Any permanent work restrictions must be processed on a yearly basis.
  • Depending on nature of the work restrictions, Jeff Tickle will schedule a meeting with the employee to review the work restrictions and job description.
  • After review, Human Resources will determine if the work restrictions can be reasonably and safely accommodated.
    • If work restrictions include work accommodations, see next section.

Work Accommodations

  • Complete Work Accommodation form with your medical provider.
    • Medical provider must sign the form or provide supporting doctor’s note.
  • Human Resources will review the work accommodation request.
  • Employee will be contacted if there are any questions and/or a need for further discussion/clarification.
  • After review and approval, Jeff Tickle will work with the necessary parties to fulfill the work accommodation request.

It is required to submit life event/status changes within 30 calendar days.

Name Change

  • Step 1: Complete the Name Change/Marital Status Update Form
  • Step 2: You will be contacted by a Human Resources Specialist to schedule an appointment.
  • Step 3: Bring your new Social Security Card with your new legal name to your scheduled appointment. This is required in order to update your legal name.
  • Step 4: During your HR appointment, be prepared to have a picture taken for an updated ID badge.
  • Step 5: HR will provide you with the appropriate payroll forms for any necessary updates.

Marital Status Change

  • Step 1: Complete the Name Change/Marital Status Update Form
  • Step 2: You will be contacted by a Human Resources Specialist to schedule an appointment.
  • Step 3: Bring in supporting legal documentation for the marital status change.
  • Step 4: If your marital status results in a legal name change, you will need to bring your new Social Security Card with your new legal name change. During your HR appointment, be prepared to have a picture taken for an updated ID badge.
  • Step 5: HR will provide you with the appropriate payroll forms for any necessary updates.
  • Step 6: Our Benefit’s Specialist will contact you regarding any applicable changes to benefit enrollment.

  • Utilize the login that you created during open enrollment. A password retrieval is available if needed.
  • Visit the site 24/7/365 to find helpful information about your benefits, insurance rates, and links to carrier sites.
  • Bookmark this site and visit any time. Content will be added throughout the year, including wellness tips and much more!

New to Lakeville Area Schools

  • You are benefits eligible the first day of your employment.
  • Please do not delay, you have 30 days from your hire date to enroll in benefits.
  • Visit https://lakevilleareaschools.hrintouch.com/
  • Select Create an Account link to begin the account creation process.
  • Once you have logged in to your new account, take time to review your insurance rate sheet found under Quick Links and the benefit information available behind each tab.
  • You will need the legal name and social security number for all of your dependents (a spouse is a dependent on your plan) and all beneficiaries.
  • When you are ready, simply select the Enroll Now Button!

Qualifying Life Event

  • What is a Qualifying Life Event? The following includes commonly recognized qualifying life events:
    • You have experienced a Change of Status in your position with Lakeville Area Schools resulting in a change to your benefits eligibility.
    • You are not currently on our plan because you were on a family member's plan at the time of enrollment and you have lost your coverage.
    • Your legal spouse and/or dependents have lost their coverage and you would like to add them to your plan(s).
    • The birth, adoption or new guardianship of a dependent under the age of 26.
    • Loss of coverage as a result of divorce or legal separation.
    • Loss of coverage due to a death in the family and you and/or your dependents were on the deceased member's plan.
    • If your loss of coverage is for any other reason, please contact our Benefits Specialist, Sara Riegel at Sara.Riegel@isd194.org to review.
  • Please do not delay. You have 30 days from your qualifying life event to notify our department.

Request Access for Non-District Staff

All forms to request access for Non-District Staff and/or Volunteers must be completed by the Building Principal/Director, Office Manager, or other ISD194 Administration. Forms not completed by an authorized official will not be processed.  Please fill out the Non-District Staff Access form to request access levels for your non-district staff. Form covers the following areas: Gmail accounts and/or Computer login accounts, Campus access, Security Cards, and/or Phone/Voicemail access.   All requests will be vetted through each Department's protocol for final determination of access rights.  If you have any question, please contact:
